Please read these forms, sign where indicated, and bring them to our first meeting. We will discuss them at our scheduled intake meeting together.
Fees
$120 is my standard rate for a 50 minute session. I will bill your insurance and you will be responsible for the copay or whatever cost your insurance don’t cover.
I do offer a discounted rate ($95) for those who prefer to submit the claim to their insurance company themselves. I will provide needed paperwork. I offer this discount due to the time and effort involved in billing.
At times I do have openings for clients with reduced means (sliding fee). Please contact me to see if I have any current availability.
$40 – for those making less than 20,000 a year
$50 – for those making less than 40,000 a year
Please remember that payment is always due in full at each session in the form of cash or check. You may also conveniently pay in advance using your credit card safely and securely by clicking below. For credit card payments, please make the transaction prior to the session.
